Initial Health Evaluation

Your initial visit is a comprehensive medical screening designed to evaluate your current health status, lifestyle, medical history, goals, and readiness for participation in the practice.

Patients are required to bring laboratory results obtained within the past 3 months, including:

• Complete Blood Count (CBC)

• Comprehensive Metabolic Panel (CMP)

• Lipid Panel

• Hemoglobin A1c

• Thyroid Stimulating Hormone (TSH)

• Vitamin D

Optional labs may include:

• Testosterone levels when clinically appropriate

Please also bring:

• Current medication list

• Medical records or documentation detailing your past medical history

• Any recent imaging or specialist evaluations relevant to your care

This visit includes a comprehensive review of your health history, lifestyle factors, current concerns, and long-term wellness goals.
